Yacovino Manoeuvre
The Yacovino Manoeuvre is a specialised repositioning technique used to treat anterior (superior) canal Benign Paroxysmal Positional Vertigo (BPPV). It is a less common but important manoeuvre for cases where the anterior semicircular canal is affected by displaced calcium crystals (otoconia).
The manoeuvre is performed by a trained ENT specialist and is designed to guide the displaced crystals out of the anterior canal, resolving vertigo symptoms.
What is the Yacovino Manoeuvre?
The Yacovino Manoeuvre involves a sequence of head and body position changes that use gravity to move displaced otoconia from the anterior semicircular canal back into the utricle. Unlike the Epley's Manoeuvre, it does not require identification of the affected side, making it useful when lateralisation is uncertain.
Who Needs the Yacovino Manoeuvre?
- Patients with anterior canal BPPV
- Vertigo triggered by looking upward or lying back
- Downbeat nystagmus observed during positional testing
- Failed response to standard Epley's Manoeuvre
- Uncertain lateralisation of BPPV
How is the Manoeuvre Performed?
The patient sits on the examination table. The specialist lowers the patient to a supine position with the head hanging well below the table edge (deep head hanging position) and holds for 30 seconds. The head is then brought to the chest (chin tuck) and held for 30 seconds, before the patient is slowly returned to an upright seated position. This sequence guides the crystals out of the anterior canal.
