Epley's Manoeuvre

Epley's Manoeuvre is a highly effective, non-invasive repositioning technique used to treat Benign Paroxysmal Positional Vertigo (BPPV) of the posterior semicircular canal. It works by guiding displaced calcium crystals (otoconia) out of the semicircular canal and back into the utricle where they no longer cause vertigo.

The procedure provides rapid relief from positional vertigo and is considered the gold standard treatment for posterior canal BPPV.

What is Epley's Manoeuvre?

Epley's Manoeuvre involves a sequence of four specific head and body position changes, each held for about 30 seconds. The controlled movements use gravity to reposition the displaced crystals, eliminating the trigger for vertigo episodes.

It is performed by a trained ENT specialist and typically provides relief within one to three sessions.

Who Needs Epley's Manoeuvre?

  • Patients diagnosed with posterior canal BPPV
  • Vertigo triggered by lying down or rolling over in bed
  • Dizziness when tilting the head up or down
  • Positive Dix-Hallpike test result
  • Recurrent positional vertigo episodes

How is the Manoeuvre Performed?

The patient starts seated on the examination table. The specialist turns the head 45 degrees toward the affected ear and quickly lowers the patient to a lying position with the head extended. After 30 seconds, the head is rotated 90 degrees to the opposite side, then the body is turned to face downward, and finally the patient is brought back to a seated position. Each step allows the crystals to move progressively toward the exit of the canal.
