Supine Roll / McClure Pagnini Test

The Supine Roll Test, also known as the McClure-Pagnini Test, is a diagnostic manoeuvre used to identify Benign Paroxysmal Positional Vertigo (BPPV) affecting the horizontal (lateral) semicircular canal. It is the standard test for detecting horizontal canal BPPV, which presents differently from the more common posterior canal variant.

The test is simple, non-invasive, and provides immediate diagnostic information to guide the appropriate treatment manoeuvre.


What is the Supine Roll Test?

The patient lies flat on their back (supine position) with the head in a neutral position. The specialist then quickly rotates the head 90 degrees to one side and observes for nystagmus and vertigo. The test is repeated to the other side. The direction and pattern of nystagmus help identify the affected ear and the type of horizontal canal BPPV (canalith or cupulolithiasis variant).


When is it Recommended?

  • Vertigo triggered by rolling over in bed
  • Dizziness when turning the head side to side while lying down
  • Negative Dix-Hallpike test with persistent positional vertigo
  • Horizontal nystagmus observed during positional testing
  • Suspected lateral canal BPPV

How is the Test Performed?

The patient lies supine on the examination table. The head is quickly turned 90 degrees to the right and held for 30–60 seconds while the specialist observes eye movements. The head is then returned to centre and rotated 90 degrees to the left. The side that provokes stronger nystagmus and vertigo typically indicates the affected ear.
